I agree that Development Dialogue mentioned is a good reference.
There was a much earlier edition which you may also find useful:
- Development Dialogue 1985 (2) 'Another development in
pharmaceuticals' had contributions from a range of countries and 
some very useful analytical pieces.
- Kanji N, Hardon A, Mamdani M, Walt G. 1992. Drugs Policy in developing
countries.  London: Zed Books. is very comprehensive.
- Chowdhury Z. 1995.  The politics of essential drugs.  the makings of a
successful health strategy: lessons from Bangladesh.  London: Zed Books/Dag
Hammarskjold Foundation.  is really worth reading.
