[indices] Dosing of Bactrim for stenotrophomonas maltophilia infection [3]

Dear all,
Thanks for the info. Bactrim is chosen as it is recommended to be the
primary choice in my hospital.
Based on the different references, the dose of bactrim range from as low as 960mg ( Trim. 160 mg/ Sulfa. 800 mg )Q12H to 12-15 mg/kg/day (trimethoprim [ so for a 60kg patient, the dose could be 720mg (Trimethoprim)/ 3600mg Sulfamethoxazole]. As the dose range was so wide, I am confused on which dose to be used especially when it comes to a patient with renal impairment who needs bactrim dosage adjustment.
Wonder if there is any other references that could clarify that?
