Dear Colleague(s)

 As you know INDICES has been operating as a discussion group for a few
 years. We want to move forward on the original objectives set for
 INDICES in Berlin in 1997. In order to achieve this, and to be sure that
 the way forward meets your needs, we would be most grateful if you would
 take a few minutes to answer the following questionnaire.

 Please would you be so kind as return your completed questionnaire to
 Leesette Turner at
 hamis@iafrica.com by 18 February 2003. [the easiest way to do this is by
 FORWARDing the email to hamis@iafrica.com and fill in the answers in
 CAPITALS. If you just press REPLY, then the message goes to the whole
list,

 All completed questionnaires will be entered into a draw. The first
 questionnaire drawn will win a copy of the publication "Daily Drug Use".
