Cefepime should be reserved as a secondary line agent for infections where
pseudomonas is of concern.

Absolutely not as surgical prophylaxis.

For dirty surgeries other agents should be available - depends on site of
surgery. Certainly, ampicillin, gentamicin with clindamycin would provide
terrific coverage plus or minus metronidazole.

If you don't control cefepime now, your pseudomonas will develop
resistance, quickly.
