I work as an emergency physician and it's not clear why you want to
use
that combination. Where I work we use ketamine IM for children.
Ketamine does not suppress respiration and giving it IM avoids the
trouble of trying to get young children to swallow the medication.
Midazolam may suppress respiration and since ketamine alone is very
effective adding midazolam does not have any advantages. My
recommendation is just to use ketamine IM.
