Dear Marc,

According to information given in MICROMEDEX trough levels of 
more than 5-10mcg/ml in neonates are toxic. Although these are 
for 8-12 hourly regimen but the Therapeutics and Goods 
Administration, Australia recommends that the same doses can be 
applied to once daily dosing regimen, except in endocarditis.
