INDICES> dexamethasone / alternative

Hi

Decadron Shock-pak Injection (dexamethasone 20mg/ml in a 5ml vial) 
has been discontinued. To use the smaller ampule at 4mg/ml in a 2ml 
amp would, in the case of treating shock at 2-6mg/kg, involve using a 
very large volume and opening numerous ampoules.
Does anyone have any alternative suggestions ?

p.s. Pertofran (desipramine) was discontinued some years ago by Ciba 
Geigy. Does anyone know of any stock any where or a generic?
