Good morning
It is my feeling that we lack a substantial amount of clinical detail
regarding this mans diabetes, ie HbA1c and if he is on insulin or
not.
Another issue is his cholesterol as well as renal function, both
impacting on his long term prognosis. Is his BP well controlled ie
120/70-75, as he is a vascularpath and all these add to his ? autonomic
neuropathy!
My opinion is that he should have his blood glucose and BP strictly
controlled as well as cholesterol, and then see if his neurological
problem improves.
Surgery is inappropriate at present, and domperidone and
metoclopramide are useful at present.
