Dear Firends
A middle aged male patient (approx.57 years) advocate in profession, married
without any issue has been suffering from Diabetes Mellitus with
hypertension for the last 30 odd years. He has positive family history of
these two ailments.
The patient has been prescribed oral hypoglycemic agent at the initial stage
which he did not take regularly but he has been taking his anti hypertensive
medications on a regular basis. [no details on these drugs - Moderator]
Since last 3-4 years he has been experiencing constipation with slow bowel
movement, he is also complaining of erectile dysfunction with loss of libido
along with the gastrointestinal disturbances. For these problems he has
consulted various physicians including Gastroenterologist, Neurologist etc.
He has also undergone various investigations like Colonoscopy, Endoscopy,
Sigmoidoscopy etc. results of which were inconclusive. In last 2 years, he
underwent Anal stretching operation on two occasions but that too failed to
yield any positive result.
Doctors concluded that he has been suffering from Diabetic autonomic
neuropathy and have prescribed him prokinetic agents like Metoclopramide,
Cisapride etc.
Few months back he went to Apollo Hospital, Chennai for consultation with
eminent Neurologist. There also he was diagonised as a case of Diabetic
Neuropathy and was advised to undergo MRI of spine and Electrophysiological
Stimulation test of different nerves. The patient is at the moment taking
prokinetic medicines, Amlodipine(anti hypertensive) Anti psychotic
medicines.
In this context, we would be glad to know the treatment modalities and also
suggest us of any surgical intervention if necessary.
