INDICES> apnea treatment in premature babies?

Dear indices friends,

I need some info on treatment of apnea in premature
infants. caffeine,aminophylline and theophylline are
used(micromedex). but the treatment should be started
when? is it 6 hours or 12 hours after the birth of the
infant.what is usual guidelines to be followed?
