The best critical review can be found on p. 827 of the last edition (5,
1997) of Kucers et al.: The Use of Antibiotics - the most authorative
textbook.
The main points are that it is very effective (in controlled trials) to
prevent sepsis, particularly due to St.aureus. It can be used on
uninfected burns; some hospitals combine it with 0.2%
chlorhexidine. In one trial there was an emergence of sulfonamide-
resistant enterobacteriae. Some say it prevents Pseudomonas
infections, but others say that since 1980 Pseudomonas has been
on the increase in treated patients.
Theoretically use of the cream can elicit sulfonamide
hypersensitivity, but precise data on incidence are not available.
Dry open exposure to laminar flow in a sterile environment is better,
but that is a theoretical option in Africa.
