I have not done a literature search but on an empiric basis you could
estimate the volume of peritoneal fluid and administer sufficient
fluconazole to well exceed the MICs of the likely organisms. In any case as
long as you don't exceed the normal IV dose for neonates there shouldn't be
a problem.
On a general point you would have to cautious with the use of fluconazole
as it is renally excreted and thus may accumulate especially in a premature
baby.
