As a clinician I have a feeling that this query basically touches a
medical ethical issue and has nothing to do with pharmacokinetics or
drug monitoring: Should one treat a patient with terminal renal
failure on haemodialysis, apparently suffering from a terminal
ovarian carcinoma, with such highly toxic oncolytics? What do the
physicians expect? If the patient had received a precious renal
transplant one should not even have considered such use. Giving a
"scientific" answer seems to me highly unrealistic. I would be very
upset if a similar question would be asked if the patient was my
wife. Humans are not experimental animals.
