Following are the answers to your queries:
1. What is the use of Methotrexate in Ectopic Pregnancy?
Ectopic pregnancy occurs when the fertilized ovum implants outside the uterus, usually in the fallopian tube. Laparoscopic surgery remains the standard treatment. In non-surgical methods, methotrexate is widely used. The management with intramuscular methotrexate may be appropriate for women with small unruptured tubal pregnancies who are hemodynamically stable.
2. If there is any comparative studies (How much effective) for the following drugs?
The comparative studies including meta-analysis and reviews are available at www.cochrane.org.
3. Why evening dose of Dexamethasone is avoided which can cause insomnia.
The answer of question is already mentioned in second part of the question. The patients who suffer from insomnia after evening dose of dexamthasone avoid the evening dose and are advised to take the dose in the morning.
4. Why Metformin is discontinued before major surgery?
Metformin is discontinued before surgery because normal food intake and normal renal function is not there. When normal renal function and food intake are resumed then metformin is started again.
