Omeprazole (4)
********

Dear Dr. Khan,

Thank you sir for writing on it at-length.

During my UG time, I was told that antacids (probably H2antagonists), if administered during the course of PPI, reduces its efficacy as PPIs are not able to isomerise at a less acidic pH. Is this a myth?

Since the effect of PPIs lasts long (for three days after discontinuation of a regular coures) and discontinuation of PPIs is associated with rebound hyperacidity, would it be logical to prescribe a combination of an H2 antagonist alternating with a PPI (H2 antag. on mon, wed, fri and PPI on tue, thu, sat )?
