Dear Manjiree,
                 One dose not have to re invent the wheel. Most of the text 
books have the standard format of a presciption order. Katsungs Pharmacology 
or Goodman and Gillmans, The Pharmacological Basisa of Therapeutics have a 
chapter on prescription writing.
            Once you open the pages of these books you would find it. I 
believe all medical students are taught this.
