Dear Dr Pipasha
In response to your question of imposing strict laws to make ADR
reporting mandatory, we are not sure that is the right approach. The
foundation and cornerstone of ADR monitoring is spontaneous reporting
wherein every physician and practitioner in the country perceives it as
part of his or her duty to report adverse reactions. This is the most
sensitive, powerful and cost effective system for ADR monitoring and
generation of signals. 20 ADR reporting and generating a reporting
culture is more easily achieved in countries with small populations.
India, with its vast population is more a continent than a country,
and thus has its own unique problems. Interestingly, even countries
with good ADR monitoring systems have low rates of spontaneous
reporting and this in fact is a worldwide problem. Improving ADR
reporting can only be done through education, motivation and improving
awareness among the physician community, who hold the key. Prior to
imposing laws, it may be worthwhile setting out achievable goals, and
periodically monitoring achievements.
