Ref.: Deuxième et troisième génération de contrefaçon de comprimés
d'artesunate de Guilin Pharma.

A la fin des années 90 on avait trouvé des contrefaçons de comprimés
d'artesunate, un anti-malaria majeur, circulant en Asie du Sud Est. Pour 38%
d'entre eux achetés dans des échoppes et des pharmacies d'Asie du Sud Est et
étiquetés comme provenant de Guilin Pharma, République Populaire de Chine,
on n'a pu détecter la présence d'artesunate. Ce qui conduit inévitablement
vers un accroissement de la mortalité et de la morbidité parmi les patients
touchés par la malaria due au falciparum dans cette région, même si on ne
peut le quantifier.

Les contrefaçons décrites en 2001 se reconnaissaient assez facilement par
l'aspect de l'emballage et de l'hologramme. Un test de coloration simple et
peu coûteux permet aussi à tout un chacun de vérifier l'authenticité des
comprimés d'artesunate.
Maintenant, ce message vise à alerter de la présence de deux nouvelles
"générations" de contrefaçons de comprimés d'artesunate, étiquetés comme
provenant de Guilin Pharma, achetés au Laos et au Cambodge et portant un
hologramme nouveau et très ressemblant, même si il est faux, collé sur la
boite de blisters.

En ligne, au format "pdf" on trouvera les documents informant des
découvertes récentes de contrefaçons sophistiquées d'artesunate, photos à
l'appui, des différents hologrammes et une description du test rapide de
couleur rouge. Le tout est disponible en s'adressant à

La première génération d'hologrammes contrefaits, décrite en 2000, n'est pas
à proprement parler un hologramme mais un autocollant facilement
reconnaissable par comparaison avec l'hologramme original. Elle est toujours
en circulation. La deuxième génération est un vrai hologramme et ne se
distingue de l'original que par la forme des montagnes en arrière plan etpar
l'absence du texte microscopique "Guilin Pharma" écrit sous "waves".

L'impression sur le blister n'est pas nette. Trois blisters ont été achetés
récemment dans le sud du Laos et un dans le nord est du Cambodge, tous
portaient cette seconde "génération" d'hologrammes. Tous avaient le même
code et les mêmes dates de fabrication et d'expiration (détails disponibles
en "pdf"). Il est très probable cependant que l'artesunate produit avec
cette deuxième génération d'hologramme portera des codes et des dates
différentes.

Sur la troisième "génération" d'hologramme, les montagnes ont le même profil
que sur l'original, mais il manque l'inscription microscopique sous "waves".
L'impression est nette et ressemble à celle du produit original. Deux
blisters portant ce nouvel hologramme ont été récemment achetés dans le sud
du Laos (pour connaître les codes et les dates voir le dossier "pdf")
Les six blisters portant les hologrammes contrefaits de 2ème et 3ème
génération sont négatifs au test rapide coloré tout comme à l'analyse par
HPLC.
Nous craignons une large distribution de l'artesunate contrefait avec des
hologrammes de 2eme et 3eme génération et nous craignons qu'ils ne passent
inaperçus des personnels de santé, des pharmaciens et des malades grâce a
leur grande ressemblance au produit original Nous vous remercions de faire
circuler cette information. Si vous trouvez des cas similaires, nous vous
remercions de nous en faire part, ce qui permettra de suivre leur
distribution. Nous avons aussi appris que Guilin Pharma fournit maintenant
des pays d'Afrique de l'Ouest comme le Togo et nous sommes inquiets d'y voir
circuler des contrefaçons. Toute information sur ce sujet nous sera très
utile.
Il est très urgent d'agir et nous espérons que cette information peut
déclencher cette action visant à combattre ce problème sous-estimé de santé
publique.

Re: Second and third 'generation' fake Guilin Pharma 'artesunate' tablets

In the late 1990s counterfeits of artesunate, a vital life-saving
antimalarial drug, were discovered circulating in SE Asia. Up to 38% of
'artesunate', labelled as manufactured by Guilin Pharma, People's Republic
of China, bought in pharmacies and shops in mainland SE Asia contained no
detectable artesunate. This has led to an unquantified but inevitability
high mortality and morbidity amongst falciparum malaria patients in the
region. The fakes described in 2001 were relatively easy to distinguish from
the genuine product by the appearance of the packaging and holograms. A
simple, inexpensive dye test allows one to reliably check the authenticity
of artesunate tablets.

We are now writing to warn of two further sophisticated 'generations' of
counterfeit 'artesunate', labelled as produced by Guilin Pharma, bought in
Laos and Cambodia with new, convincing and very well crafted but fake
holograms attached to the blisterpack.

'Pdf' files of documents warning of the recent discovery of  sophisticated
counterfeit artesunate, with photographs of the different holograms, and a
description of the Fast-Red dye test, are available from

genuine and fake holograms are also posted on the Shoklo Malaria Research
Unit website (www.shoklo-org.com).

The first generation fake 'hologram', described in 2000, is not a true
hologram but a sticker and easy to distinguish from the genuine hologram.
This is still in circulation. The second generation hologram is a true
hologram and only appears to differ from the genuine hologram in the shape
of mountain outline and the lack of themicroscopic legend 'Guilin Pharma'
printed below the 'waves'. The printing on the blisterpack is not clear.
Three blisterpacks were recently bought in southern Laos and one in NE
Cambodia with this second generation hologram. All have the same code, and
manufacture and expiry dates (details in pdf file). It is likely however
that artesunate with this second generation fake hologram has or will be
made with different dates and codes.

The third generation hologram has a mountain outline similar to the genuine
Guilin product but lacks the microscopic legend 'Guilin Pharma' printed
below the 'waves'. The printing on the blisterpack is crisp and similar to
that on the genuine product. Two blisterpacks bearing this new hologram were
recently bought in southern Laos (for printed codes and dates see pdf file).

All six artesunate blisterpacks with the 2nd and 3rd generation fake
holograms were negative for artesunate by the Fast Red Dye Test and
contained no artesunate on HPLC analysis.

We fear that fake artesunate with the new sophisticated second and third
generation fake holograms are widely distributed but, because of their
similarity to the genuine product, that they are unrecognised by
pharmacists, health staff and patients.
Please disseminate this information. If you find any similar fake artesunate
we would be very grateful for the information - so that we can map the
distribution. We have heard that Guilin Pharma artesunate is now sold in
some west African countries, such as Togo, and we are concerned that fakes
may also be circulating. We would be very grateful for information on this.

There is an urgent need for action and we hope that this information might
prompt action to combat this under recognised serious public health problem.
