E-farmacos: Histoacryl (cont.)
-------------------------------------------------------------------------

Hola Alicia,

Topical agents used in the repair of minor dermal lacerations in a
pediatric emergency department

AB: An overview of the use of topical agents to minimize pain and
anxiety prior to suturing of dermal lacerations in pediatric patients is
presented. The preparation of the patient, the use of oral analgesics
and sedatives, and local anesthesia with lidocaine (lignocaine),
tetracaine, or cocaine with epinephrine (adrenaline) or phenylephrine as
adjuvants are discussed. A variety of administration routes are
discussed, including infiltration and topical application. A tissue
adhesive (Histoacryl) that polymerizes quickly
on contact with tissue fluid and has been found to be useful in
replacing the need for suturing in children with simple dermal
lacerations is described.

AB: The role of cyanoacrylate adhesive lateral tarsorraphy as an
alternative to lid-taping or surgical tarsorraphy was studied in 9
patients with exposure keratopathy secondary to facial nerve palsy who
received
lateral tarsorraphy with butyl cyanoacrylate adhesive (Histoacryl-blau;
enbucrilate), with adjunct treatment of regular wetting agents and
topical ointment at night. The 9 patients received 25 adhesive induced
tarsorraphies, lasting for a mean of 9 days (range 4-25
days)/application. The duration of the palsy needing treatment ranged
from 5-56 days. Follow-up was for 2-12 wk. In every patient the
condition of the corneal epithelium improved. Four patients complained
of mild stinging as the glue dried and one tarsorraphy caused discomfort
when a lash entrapped in the glue came into contact with the cornea.
Seven patients regained satisfactory orbicularis function, allowing
discontinuation of treatment. It was concluded that cyanoacrylate
tarsorraphy is a useful additional non-invasive measure for the
treatment of short-term corneal exposure.
