MSF have put a description of their rationale and approach (including a
comparison with the effectiveness of other forms of supplement) on their

It's worth reading.
It is also worth looking at a site belonging to the AED (Academy for
Education and Development) food and nutrition site, where there is a
manual on therapeutic feeding at

- chapter 11 of the manual explains about the possibilities of local
production of the paste/spread from milk powder, peanuts, vegetable fat
and sugar plus vitamins & minerals. It can be provided in foil sachets
or in plastic pots. Apparently it is like an enriched peanut butter.
FANTA project and MSF report on using it for <severely> malnourished
young children under the age of three.
There is also a potential use for children who are severely wasted due
to HIV-related disease and need therapeutic food supplements to help
them recover more quickly when they are started on antiretroviral
treatment. But I haven't seen anything written on this yet - if anyone
has, it would be helpful to hear about it.
