Dear Idris,

It was interesting to read your comments. Though I wish to clarify on the following: I do not question in anyway the benefits of FDC's, especiallly in paediatrics. Anybody who is dealing with paediatric ARV patients at operational level will agree that FDC's for children are a blessing; we are having paediatric FDC's for TB treatment since some time now and we really appreciate these dosage forms.

The new Namibian ART guidelines for adults changed from stavudine to zidovudine as 1st line treatment because of possible side-effects and toxicities that occurred, especially during longterm use. In paediatrics just the opposite has happened and we will have stavudine instead of zidovudine as 1st line treatment. Because of the latter fact I am interested whether other settings have experience with longterm use of stavudine in paediatrics.

I am looking forward to getting some more comments.
