E-DRUG: Intraventricular dose of vancomycin?

Dear Colleagues

A patient in our neonatology ICU requires intraventricular administration of vancomycin . As the use is unlicensed would any body send me hospital protocol or any other authentic reference of its dosing so that the patietn may receive the drug. Any comment on use of vancomycin through this route and experience would also be appreciated.
