When I was researching this very topic five years ago for another
organization, I found one company producing generic ivermectin for human
use: Shinpoong Pharmaceuticals in South Korea.  I don't know if this is
still the case.  

At that time there were also no generic ivermectin producers in the more
lucrative veterinary market, despite being off-patent in the US since
1997.  But as someone already pointed out, the veterinary market has
since opened up to competition.  

In the market for human-use, Merck's donation program has the potential
to cause serious (if unintended) distortions in access to ivermectin, as
this situation in the Pacific islands may illustrate.
