I had been involved in similar discussions sponsored by Roche and I had
aired similar concerns as expressed by Dr. Ball. In developing countries
out-of-pocket expenses paid by a patient may be quite expensive as far as
the patient is concerned. Besides there is a general tendency to believe
that once the symptoms disappear then the disease is cured. This belief is
so common amongst many patients that it leads to incomplete treatment and
development of resistance. OPAT is avery sound science but it is not
cost-effective in most developing countries. We need to revisit the concept
of DOTS in the case of TB.
