WHO Drug Action program has an old document "Development and 
implementation of drug formularies." You can go through that. 
Additionally there is a book Managing Drug Supply  by Management 
Sciences for Health. It also gives some details on developing and 
using hospital formularies. If you want, I can send you a copy of the 
particular chapter on your mailing address.
