E-drug: Myasthenia gravis (MG)

Dear e-druggers,

Can anybody kindly supply me with any information on:
1.	 MG prevalence 	and treatment in developing countries?

2.	In South Africa, the cost of the only registered oral cholinergic 
	(pyridostigmine bromide) is approximately US$0.22 per 10mg tablet in 
	the private sector. Are cheaper alternatives/equivalents available 
	internationally?

3	Strategies to facilitate the early diagnosis and proper/affordable 
	treatment of MG/rare chronic conditions?	

Information on the above will be sincerely appreciated.
